Chapter 5

Chapter 5 — The Heart Is Not a Metaphor

Marija arrives at Dr. Petrova's office for the early appointment. Petrova reveals that Marija's cardiac enzymes and inflammatory markers show patterns that don't match her previous remission records, but in an unexpected direction: her heart tissue appears to be adapting rather than deteriorating. Petrova questions Marija about the intensity and duration of her fasting and running, probing for details that might explain the cellular activity. Marija describes the signal she's been feeling, and Petrova listens with unusual scientific intensity before admitting she's been researching autophagy on her own. Jonas calls after the appointment, and Marija deflects his concern. The chapter ends with Marija processing the idea that her heart has become the subject of something she doesn't yet understand, sitting in her car outside the clinic, aware that the conversation is not finished.

I wore the running shoes to the appointment. This was not a statement. I didn't lace them up as some act of defiance or symbolism or narrative convenience. I wore them because my feet were in them by seven-fifteen, heading out the door for the walk to the clinic, because walking calmed me and driving did not, and because the shoes were already there. Sometimes the most meaningful choices are just the ones you were going to make anyway.

The morning was cool. October light, thin and particular, the kind that makes every surface look like it's being observed. Dr. Petrova's office was on the third floor of a building that had been renovated just enough to feel modern without losing its institutional smell. That particular blend of floor wax and anxiety. I'd spent enough hours in that hallway to know which ceiling tile had the water stain shaped like Portugal and which chair in the waiting area had the slightly tilted leg that made everyone who sat in it look vaguely seasick.
